Patient Activation through Community Conversations (PACC) is a program where people share their experiences so others can take away important information that would benefit someone as a professional patient advocate, caregiver, someone recently receiving a new diagnosis or entering the healthcare system. All experiences should be related to being a patient or helping / supporting a patient. PACC encourages finding solutions through what has worked for others.
Camera and microphone are strongly suggested for face-to-face conversation.
Reminder: No advice is to be given unless asked – Medical advice is never given. This is not about your health, it is about your health CARE the journey to the best health.

Each meeting is on the first Wednesday of every month.  7:30 PM
